Any time a recruit stays overnight on campus, USM needs the following info on file as soon as possible. If any information changes after submitting this form, please re-submit.

All recruits must complete the Overnight Guest Agreement Form
This form requires the signature of their parent/guardian and their USM Student Host. Completed forms must be turned in to Student Life.

If you haven’t confirmed a host yet, please email the host’s name to StudentLife@stmary.edu once confirmed – no later than 24 hours prior to the recruit’s visit.
