I have received my offer of admission for the Class of 2028 cohort with the University of Saint Mary Doctor of Occupational Therapy program. At this time, it is my intent to enroll for this program.
Please accept this letter and my $400 deposit paid via your website as my commitment and request to formally reserve my spot in this upcoming class. I understand that this is not legally binding, but it is a formal declaration of my intent to enroll in the Doctor of Occupational Therapy program at the 糖心视频. I understand that the Doctor of Occupational Therapy Program expects to receive this declaration of intent or communication of my declination of this offer by or before the date indicated in my offer email. If I have not returned it and my deposit by or before that date, my seat in the OTD class may be offered to another applicant. Additionally, I understand that I will forfeit my deposit if I choose to not to enroll in the program after submitting this form.
